MEMBERS RESPONSIBILITY 

Each Airman is presumed fit for duty and it's his/her responsibility to "be ready" Each Airman is responsible for promptly (within 72 hours) reporting any new medical or dental condition illness, injury, disease, operative procedure, or hospitalization) or not previously reported, or any change in a medical or dental condition that has been previously reported, to the medical group. If you have been evaluated by the VA and/or have a VA disability rating, ensure the Medical Group is aware of the injury/Illness.

Each Airman is responsible for meeting his/her scheduled appointments and should inform his/her supervisor of any upcoming scheduled appointments. Airman should make all attempts to resolve medical conditions in a timely manner. This includes, but is not limited to, attendance at all appointments, active participation in rehabilitation, and using medications as prescribed by their health care provider.

Each Airman with a known medical or dental condition is responsible for submitting all medical and dental documents for treatment obtained through civilian sources within 60 days of treatment. Airman who refuse to comply with the request for medical information will be considered medically unfit for continued military duty and referred to his/her immediate commander for administrative processing.

Airman are responsible for completing the AF Periodic Health Assessment Questionnaire (PHAQ) annually and for monitoring and maintaining currency of his/her IMR requirements. IMR status can be monitored using MylMR

PHA-Q Instructions

Go to My IMR/ASIMS Click Air Force Web Health Assessment. Start PHAQ.

Medical Care:

-Ensure you report surgeries, minor procedures, lab testing, and radiology since your last PHA

Medications:

-Report current medications including previously reported medications you are still taking

-Include prescriptions, seasonal medications and medications taken intermittently

-Over the counter (e.g.: Tylenol, Ibuprofen, etc.), vitamins, supplements, etc.

Family History:

-Ensure family history is as accurate as possible and include previously reported family history

Dental Form DD2813

In order to process your dental documentation on a timely fashion, please note the following:

1. Please ensure the DD2813s are completed in full; i.e.: provider license number, signatures, X-ray and exam dates, etc…
2. The member’s SSN is not required. To limit PII, we access the member’s information from our systems.
3. Instruct the members to submit their 2813s through their respective UHMs.
